HOW TO ORDER

CANADIAN DESTINATIONS – Paid in Canadian $$$

          1.      Print your name, complete address and Postal Code at the top of the order form.

2.        For each publication, enter the Publication #, quantity, publication name and price in Canadian funds in the appropriate columns.

3.        Calculate the total publications cost in Canadian funds.

4.        Calculate the P & H per the Canadian schedule below.

5.        Calculate the total remittance in Canadian funds.

POSTAGE AND HANDLING - Within Canada

1. First item to Ontario or Quebec: $6.00

2. First item to Manitoba, Saskatchewan, Alberta, Nova Scotia, New Brunswick, PEI: $6.50

3. First item to British Columbia, Newfoundland & Labrador, Nunavut: $7.50

4. Each additional item: $2.00

FOREIGN ORDERS OUTSIDE CANADA

1. As per 1, 2 and 3 in Canadian destinations.

       2.        To convert publication total cost to US funds, multiply by .90 (IE $20.00C = $18.00US)

3.      All P. & H. charges are in US $$$ per foreign schedule below and are not to be discounted.

4.        Calculate the total remittance in US funds
